NotAuthorized exception not defined

The "NotAuthorized" exception (which is to be used in authorization callbacks) is not defined.  I propose to use the "
AuthorizationFailed" exception instead.

Also, authorization callbacks MUST be called, and completed, bofore the associated 'open' method (or so) can 
successfully return.  The open() MUST THEN also cause the "AuthorizationFailed" exception.
