following is how i modified the document.

I added author information. 
Corresponding auther, me, only has e-mail address.

> - The first section of the Appendix has some unusual line breaks and capitalization.  Please check this and update as needed.  Particularly lines 
277-282.
The first line with asterisk means name of category and the second line
without asterisk means name of actual use case. If the use case include
name of company or project, it has capital.
So, line breaks have meaning. But I changed it by using indentation.

> I suggest also updating some of the formatting.  There are many cases in section 4.1 and the Appendix where there is an asterisk in the leftmost 
column, then an indentation, then a bullet or list number (such as a, b, c).  Consider dropping the asterisk, and instead insure consistent 
indentation and numbering/bulleting (see also section 1.1, which is not indented like the other list items).  Consistency will make the document 
easier to read.
Delete astarisk and use a) etc. and no indentation for all items.
